No answers I'm afraid, but a question I have been asking for over 25 years! To help get to the bottom of this I just a fantastic chat with Gill Kernick author of "Catastrophe and Systemic Change: Learning from the Grenfell Tower Fire and Other Disasters"
A rather shocking and interesting book. The first half is about the astonishing catalogue of errors, ignoring concerns, corruption, incompetence, systemic disfunction and more that lead to the Grenfell Tower fire. But the second half catalogues some of the learning to help with future disasters. Really insightful, practical, useful and well written. Below is Gill's Grenfell Model for Systemic Change. She is interested in having anyone explore and use this.
We found lots of commonality between my trust project and her work. Notably, a focus on being trustworthy and the most valuable finding of my trust work which is the importance of taking seriously the views of the people you, err, don't take seriously.
